Job Description
We are looking for highly skilled clinicians with a minimum of 1 year experience in a fast pace 911 system or commensurate combined experience to provide on demand acute care management in an in-home setting to enrolled Medically in Home patients. As a Community Paramedic you will be responsible for the in-home assessment, interaction, and hospital outlined treatment plan of those patients enrolled in the Mobile Health Program while interfacing directly with the nurses and Physicians responsible for their in-home care program via telemedicine technology. This role will also be responsible for helping the patients with chronic disease management and education, post hospital discharge follow up to prevent hospital re-admissions, ensuring care plans are being followed and medication administration, setting up/tear down of hospital tech kits for patient interaction with hospital staff. Care includes iStat point of care lab testing, advanced medication administration and treatment including antibiotics, advanced cardiac medications, 12 lead, blood draws/blood cultures, IV placement and maintenance, foley/straight cath placement, wound evaluations and much more.
Position Responsibilities

Provides skilled, professional, and compassionate mobile integrated healthcare to enrolled patients.

Conducts post-hospital release follow-up care including, but not limited to home safety/living condition evaluations, medication reconciliation, wound care, vital signs, and medication administration. Able to deploy or trouble shoot patient monitoring equipment in the home.

Transport, set up, troubleshoot, maintain, recover/decon and educate patients on the required in-home Tech Packs for the patients.

Observes, records, and reports to physician/nurse patient’s conditions and reactions to drugs, treatments, and significant incidents.

Examines, screens, treats and coordinates health services for patients which may entail point of care laboratory testing based on paramedic’s education, training and competency

Conducts patient education, includes diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessments, injury evaluation, geriatric frailty visits, and nutrition.

Functions as nurse/physician/social worker and hospital extender through advanced protocol utilization

Coordinates appointments, additional home health visits and follow up with providers and facilities as appropriate/ordered

Comprehensive knowledge of principles and processes for providing customer services, including setting and meeting quality standards for services and evaluation of customer satisfaction. Ability to handle a variety of customer service issues with tact and diplomacy in a confidential manner

Ability to communicate ideas effectively, including the preparation of reports and logs. Ability to listen and understand directions, information and ideas presented verbally and in writing

Attends training sessions for new and continuing education, as required.

Perform other duties as assigned depending on the community need and/or home situation.

Performs all job duties and responsibilities as described for Paramedic including but not limited to providing BLS or ALS to patients in an emergent first responder capacity in collaboration with local EMS agencies.

Performs related tasks as required.

Education, Experience, and Special Requirements

Minimum 1 year experience at the Paramedic level, in a strong 911 system

Current EMT-P license, CPR, PALS, PHTLS and ACLS cards

Strong desire to continually improve your skills and knowledge

Critical Care Transport, Community Paramedic Certifications preferred

Ability to speak/navigate multiple languages, Spanish, Vietnamese preferred
